It is our goal to provide the best possible care to all our patients, in order to achieve that we ask for your assistance in implementing the following guidelines.

Narcotic / Control Substance Prescriptions:

  • Refill requests for the medication, which falls under narcotic will require re-evaluation of the patient health situation.
  • We require all our patients who need narcotic medication to sign a pain management contract; this contract determines strength, dosage and the frequency of the medication /s.
  • We strongly discourage early refill request for any control substance or narcotic prescriptions.

Patients with Chronic Illnesses:

  • Patients suffering from chronic pain will first be assessed by our pain management physician to prescribe a treatment plan. This treatment plan then can be followed under the supervision of your primary care physician.
  • We expect our patients suffering from chronic health problems such as diabetes, hyper tension, HLD, thyroid, renal or liver problems to please visit us at least every three months to maintain proper care.
  • Patients starting on new medications for chronic health management will require follow up visit every 2 weeks to assess any medication side effects.

Diabetic Patients:

  • Diabetic patients, who are on insulin shots, must see their doctor at least once a month for proper monitoring.

Lab Work:

  • New patients must see their doctor to discuss their lab work, as this cannot be discussed over the phone. Existing patients with normal lab results however will be informed over the phone.
  • Patients with abnormal lab results must schedule an appointment to discuss them with their physician.
  • Abnormal diagnostic evaluation results will be discussed in a follow-up visit. (cxr, u/s etc.)
  • We recommend our patients, taking medications for diabetes or thyroid problems to get their lab work done every 3-6 months.
  • Patients on cholesterol management plan will need their lab work done once every six months.
